When we wrote the introduction to the first edition of Understanding Health Policy: A Clinical Approach in the early 1990s, we characterized the health care system in the United States as a "paradox of excess and deprivation," borrowing this phrase from a published article of that period (Enthoven and Kronick, 1989).
